New patients are always welcome.

Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule.  Everyone at our office is at your service.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy on your part will make it possible to give your appointment to another patient.

Emergencies

Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week.  Call (909) 793-3544 and Dr. Collins will be contacted immediately.

Health Insurance Plans

The benefits paid by insurance companies for plastic surgery vary greatly from carrier to carrier and plan to plan.  Therefore, we make every effort to determine in advance if insurance coverage exists.  We ascertain the projected insurance payment and the required co-payment.  We do this because we believe you need to be as informed as possible before surgery.  

We know you realize that you are ultimately responsible for the full payment of your account, but we have found that our knowledge and experience can be an important factor in assisting you to collect your maximum benefits.

Financing

Most medical insurance plans will not usually cover cosmetic procedures.  Our office is able to assist you with a financing program for elective medical procedures.  Patients may be pre-approved for financing with a low-interest monthly payment plan.  Our office manager would be pleased to provide you with further details.  We also accept most major credit cards.

 

Telephone Calls to Dr. Collins

If you have medical questions which only the doctor can answer, a message will be taken and your medical file will be given to Dr. Collins who will return your call as soon as possible.  If your question is non-medical, a qualified staff member will be pleased to assist you.
